an employee of PPLE Healthcare, a division of PPLE Group. (PPLE Group or the Company), understand that PPLE Group will, in line with the Protection of Personal Information Act, 4 of 2013,(“POPIA”) that regulates and controls the processing, including the collection, use, and transfer of a person’s personal information have the right to collect, process, store and use my personal data (and other sensitive or confidential data) as identified herein for the performance of my employment contract and other lawful purposes including but not limited to personnel administration, staff communication, health management scheme, payroll services, tax requirements, and other related purposes in line with the PPLE Group Data Privacy and Protection Policy.
I am also aware that the personal data required by PPLE Group for the purposes mentioned herein include: my name, gender, nationality, date of birth, physical and postal address, previous experience, skills and educational qualifications, convictions, next of kin and their contact details, disability information, medical report, bank details, business and personal phone number, passport curriculum vitae, biometrics and any other related data that might be operationally required from time to time.
I am aware that PPLE Group may share my personal data, without any consent, with its clients, affiliates, agents, regulatory authorities, or third-party service providers, within or outside South Africa (Imperial Logistics International Head Office, Partners and Agents) in legitimate furtherance of my employment relationship or in compliance with provisions of applicable laws. I am also aware that PPLE Group will retain the personal data as long as necessary for the purpose for which it is collected.
I am also aware that I have the rights to:
a) request for and access my personal data collected and stored by PPLE Group.
b) withdraw consent at any time with a legitimate reason.
c) object to automated decision making.
d) request rectification and modification of my data kept by PPLE Group.
e) request for deletion of data.
f) be informed of and entitled to provide consent prior to the processing of data for purposes other than that for which the personal data were collected.
g) request the movement of my data by PPLE Group to third parties.
h) request that PPLE Group restricts its processing of my information.
I also confirm that I have read and understood the PPLE Group Privacy Policy, a copy of which is available on request to WCHealth@pple.co.za or from your direct supervisor.
DECLARATION AND INFORMED CONSENT
The Protection of Personal Information Act requires that all personal details supplied must be
complete, not misleading, accurate and up to date. It must be properly maintained as well as
protected. Whilst we, as a company, take responsibility for the processing and the protection of the information, it is your responsibility as a data subject to ensure that the information is complete and accurate and that you advise us of any change of personal details, so that is can be accurately maintained.
Furthermore, should any of the Personal Information belong to any of your dependents and/or beneficiaries who are underage, you in your capacity as their legal guardian and competent person give PPLE Group the appropriate permission to process their Personal Information for the purposes for which these details were given.
“Data Subject” means you, the person who will provide PPLE Group with Personal Information and who consents when providing such Personal Information, to PPLE Group for the use thereof in accordance with this Employee Data Consent Form.
I, the Data Subject confirm that the Data Subject’s Personal Information, provided is accurate, up to date, not misleading and is complete in all respects, save where same may change and then in such an event, I the Data Subject undertake to advise PPLE Group of these changes.
In consideration of all the information stated herein that are within my knowledge, I hereby consent to the collection, processing, use and transfer of my personal data for the purposes stated herein and to satisfy the requirements of the Protection of Personal Information Act 4 of 2013 (POPIA).
